Where can you get a 'second chance' bank account?

Does anyone have any information on the second chance checking account offered by US Bank?

I am on Chexsystems and am in need of a checking account. I heard a rumor that US Bank offers a second chance checking account however they impose harsh stipulations on the account. Does anyone have any information on the type of second chance checking account offered by US Bank and what the stipulations are for having one? Any chance I can get a regular account while on Chexsystems with US Bank? I have been on Chex since April of this year and have significantly cleaned up my life since the time that I was placed on there due to having an overdrawn Wells Fargo account in the amount of $500.00. Any input would be much appreciated. Thanks!
