How to motivate myself foremen playing sports. What's a good confidance boost?

Alright, so here's the deal. I would say that I am a deceantly good player. In jr high I got MVP all three years. Now when I am about to play tho, I tend to sike myself out and think negatively. I lose all my confidance and then end up playin really bad if playing at all! This never use to happen to me, and it's weird. I love basketball with all my heart and I want to keep doing well! When I'm doing not so well my brain keeps being negative and I keep ******* up even more. Basketball keeps me calm and I have a great passion for it but I feel like I'm not doing good enough. I KNOW Im not reaching my full potential. What can I do to motivate myself and keep my confidance up? Please oh please answer. This is a very deep topic for me, I don't want to lose my passion. When i am confidant I'm an all star, I score the most baskets and play really well( barley sitting on the bench) recently its been the opposite. Please, help, me :(
