  • Dear: To whom it may concern at the Appeals Board, My name is ABCDEFGHIJKLMN and my SSN# is 000-00-0000 and my claim case # is 00000000 I would like to dispute and request for (re)) redetermination and request a hearing appeal. This is personal statement. I have been through many struggles in my life and I am not blaming techs. Company or the UIA of Michigan. I have lived in Ingham county Michigan for thirteen years, I have worked for techs. Company for twelve years from 00/00/98 to 00/00/10, and my job was a very important part of my life. I was a very hard working employee and I have always been punctual reliable and always on time to ensure that I am there when I am required. I never wanted to resign from techs Company, as my dream plans were to work for techs until I retire however unfortunately things do not always go according to plan. Really appreciated and respected working for techs. Company but unfortunately it did not happen that way and I really did not like the way it ended. My family's relocation was a very important decision. Relocating my family was very important to us all. Due to my situation, my wife and I are married with five beautiful children. In 2001, my wife and I separated and since then I have really missed my family I have never stopped supporting my children . In April 2010, my wife and my kids moved to CA and in a result because of this, I became very sick. I became depressed, anxious, and very stressed-out with anxiety. I have tried to talk to my wife to reconcile after many days of pleading with my wife and after a long negotiation she finally agreed to reconciliation. I have even suggested that she move back, I offered to pay her all her expenses’ to move back to MI however for some reason she disagreed about moving back to MI and she does not wish to leave California. Sadly, I informed my Company techs in July/2010 I gave my Company techs a two weeks’ notice that I am resigning on Aug/00/2010, however I did not state to techs Co. my personal reasons, my statement was I am resigning and moving out of state. it was in the middle of Aug/10 I immediately moved to CA. on Aug/2010 and started looking for new employment. I never considered applying for unemployment benefits and I never collected unemployment for being fired, resigning, or permanent laid off. I have always been a very independent worker and a very hard working person for thirteen years and I do not like to depend on someone else for help. In this regard, I am going through a lot of hard personal times and difficulties. To save my marriage and to work towards my family reconciliation, I resigned my job in Michigan on (Aug/00/10) and rejoined my family in California. Having looked for work unsupported for 12 weeks, a qualified caseworker advised me to apply for unemployment. I followed their advice and applied for unemployment benefits to which my application has been denied. I am seeking advice from you in regard to if the Appeals Board can rule on the applicability of any part of the legislation concerned allowing for compassionate grounds? I ask in all humbleness for the Appeal's Board to reconsider my case in light of any new legislation that may have been overlooked in my first filing. I am actively seeking work and I will continue to look for work. I have registered and enrolled in cal work for jobs search, I have responded to many classified advertisements I have applied on many employment websites I have applied at numerous online agencies including, but not limited to, monster.com, nbty.com, kellyservicess.com, randstad.com, and shimano.com. I am actively looking and continuously applying for jobs in the classifieds. I am trying my absolute hardest and to the best of my ability to seek and gain employment to be with my family however, I am having some difficulties in gaining that employment. I strongly desire to again be with my family, but I have been unsuccessful. Please reconsider my application for unemployment benefits. I thank you and my family thanks you. I (ABCDEFGHIJKLMN) acknowledge that all the information provided on this document is accurate and true representation to the best of my knowledge. Sincerely _________________ ABCDEFGHIJKLMN
