It is appropriate to thank your mentor for helping. It is NOT appropriate to give a gift for a 'great grade' for your work! That smacks too much of buying a grade! The only appropriate way to refer to the grade in the thank you note that will accompany your gift is a remark like, "Your guidance and help was instrumental in helping me to achieve a high mark in the class." An appropriate, yet inexpensive, gift might be flowers, a plant, a book, a journal...or the like.
