Where do they sell Amazon.co Gift Cards?

Question about places who sell gift cards to other places?

say CVS sells a gift card to Starbucks or McDonald's, is there a set percentage each business gets, or does it differ depending on which type of gift card is sold. So if someone purchased a McDonald's gift card, how much money would CVS get (i know certain places charge just to activate the card, but its rare) but is there a set percentage of profit given or would that differ based on ether where the gift card is being sold, or what company the gift card is selling for?
