Why is my HP Toolbox not working?

HP NVIDIA Quadro NVS 450 Video Card, 3 of 4 Monitors working?

Have been running four (4) monitors on my HP elite desktop, Windows 7 64-bit Home Premium for a year without any issues. Just last week lost the 4th monitor, loaded latest NVIDIA driver, flashed the VBIOS from HP, switched cables around to see if the same outlet port was the problem, made sure all four monitors work. None of these have worked. Looked on HP's site about the card and noticed that Windows 7 64-bit Home Premium was not supported, but was not an issue before. No one at HP could tell me who actually manufacturers the card! Not running all the same brand monitors, two (2) are connected DVI/HDMi and two (2) DVI/DVI. Any ideas or has someone had a similar problems? Note, again that all 4 monitors were working before Any feedback would be greatly appreciated Derek
