Is survey scout a scam?

How to remove scam survey from embedding itself in my URL?

• Okay, so sometimes on tumblr I make links on my page. The theme I use gives me the option to add extra links. So say I had '(url).tumblr.com/tagged/music' and so then people could access music I've posted/reblogged etc. Anyway, I was clicking through my links the other night and noticed that one of them redirected me to that fake ass youtube survey saying "You've been selected from (my region) to take part in our annual survey. This will only take 30 seconds of your time and will enhance user experience. Upon completion you will have the opportunity to get a Galaxy Note, an iPhone 4S, or an iPad 2." Obviously a freaking scam. But the thing is, I thought it may have just bee a once off. I even changed the URL around and the survey has still embedded itself in my URL. This also happened on one of my other blogs. WHY? Can someone help me and tell me if there's a way to remove it and how?
