I was enrolled in a FSA for childcare in 2008. Do I still need any forms from my childrens provider before I can file my taxes?

YES, you need to know how much you paid the day care provider and their info: name, addy, SS or EIN. If you do not provide this, the amount of daycare subtracted from your income will be added back to income on your tax return.
