When my daughter was born, I made the mistake of giving her my boyfriend's last name. Now I want to change her last name to her father's. Is this possible and if so, how do I go about changing it?

Take it your (ex-)boyfriend experienced an enlightenment? You don't give a child a surname, they take the name of the father or the mother if that is unknown. You can change the name by deed-poll upon application, the consent of both parents will be required in the case of a minor.
