It says first and most obviously that crooks and criminals often get away with their misconduct, especially in domains that are not designed to resist fraud. Second, it suggests that scientific integrity is less esteemed and/or adhered to in some places than in others. Third, it suggests that some publishers and journals have lower standards for publication than others. While even the highest-status journals (e.g. Nature) can be taken in by fraud, and indeed there is a tendency to give more credence to the work of those you know and like than to "outsiders," I believe the tone of this question is misguided and misleading. The primary reason that a paper is not published in a high impact journal is rarely because the authors are not part of a secret club.
