What are the problems with both internal or external collaboration in software development teams?

    For me, the hardest part is making sure that the involved parties are all talking about the same thing.  When there's an issue with communication, nine times out of ten, it's simply because we're talking about subtly different things. Some others: Collaborating with someone in the same building, same city, same state, or even same country is relatively easy.  But how do you make it easy to collaborate with someone on the other side of the planet? How do you collect the involved programmers' knowledge?  Every company I've ever worked for has had a wiki of some kind that was never updated with proper information. How do you prevent groupthink?  That is, how do you make sure that everyone agrees with the team's decided course of action because it's the best course of action and not because they just don't want to rock the boat or step on anyone's toes?

From my point of view, the internal problems are low self-motivation, lack of sel-organization and sometimes, even lack of professional knowledge and big ambitions. The external ones are the absence of a reliable and effective project management tool with excellent team collaboration options; not efficient PM's work e.g. task delegation and resource management. All the problems can be solved if the team member are really interested in solviong them. For exmaple, the easiest point to solve is to buy a decent project management tool and you are likely to get rid of practically all external team collaboration troubles.
