  To me the most relevant and  impressive things on your list are the two conference papers.  PhD admission is all about research.  To figure out where to apply, read the literature to discover who the top researchers are in your specific subfield, and where they are.  It's not so important where you go as who you work with.  Once you've figure out a few places where you should be, add in a couple of other places that have some expertise in your area, and you're set.
