It's quite a pain in the neck.  First you have to find a company in Italy willing to hire you, which probably requires you to go to Italy to interview.  Then you have to go back to the US and apply for the work visa at the Italian consulate.  Then you wait.  Then you go back to Italy and visit the "questura", which nearly always has an unruly mob of other foreigners trying to get in to talk about their own visa problem. It's a long, expensive and frustrating process.
