    You can't. You shouldn't be asking an NP for this, you need an actual MD. Your boss writing such a letter means nothing, as your boss is not qualified to diagnose a medical condition. Disabled means you can't work. Not you can't do your current job. You can't work at all.

I know a little about this senerio, As i,m on SSi disability, Unless you have a doctor who does a complete work on you , and your going to need more than one doctor for this.It will have to be determined that you are unable to do ANY type of work, If you are capable to do work behinds a desk, or anything that does not require physical exertion, then you are not eligible for any type of disability, this is only given to people Who are unable to work period.

It's difficult to understand what you are asking. Do you have back problems AND some other kind of chronic condition? Or is the bank problem why you think you should get SDI? SDI is for people who have a sudden illness or injury or their chronic condition has worsened. It's not for people who have a chronic condition and there has been no change to that condition. Based on what the NP is saying: she agrees it's a disability, which is why you would need to ask for accommodation. But she doesn't feel it falls under any category making you eligible for CA SDI. If you don't have a work related injury, if and when a workers compensation case manager calls you, tell them exactly that. I do not have a work related injury. I'm confused why the employer filed the case. They will probably have you sign something stating that it was incorrectly filed. You had x-rays and an mri done a couple of months ago. The best place to start with further medical treatment is follow-up with that doctor. Either that person can help you or refer you to someone else.

You must get a doctor to sign you off from work. There is no other way. Tell the doctor your problems and why you have to stop working at your current job. The doctor has the discretion to sign you off for a month to start up to a year on SDI. Cancel your Workmans Comp claim if you were not injured on the job.
