It probably won't hurt to bad. It's all a mental thing. If you think it's gonna hurt really bad and you're focusing on the pain it'll be 10x worse.

The closer the tattoo gets to a bony area, the more it hurts. Just because it's not going to be directly on your bone doesn't mean that they won't be near the nerves for that area. I'd say you should expect it to hurt more than you middle/upper back tattoo. I wouldn't be able to say how much more but at least I think it will be worse.
