You need to either have a radiator shop rod out (clean the tubes) the radiator core or replace radiator. The radiator isn't able to do all the cooling engine needs and turning on heater adds a little more cooling. The radiator probably hasn't been inspected internally in 16 years, and you've described classic symptoms.
