  In my school, we had a particular teacher who was responsible for collecting the forms for each grade level (9, 10, 11, 12). The log forms were in a bin in the guidance counselor office. You had to record your hours as well as have a recording at the place you helped out. So basically, if I am remembering correctly, you needed to get a signature from the person you worked under on the log form and write where you worked. Some people I went to school with worked at 5 or 6 different places to get their hours, so they wound up having 6, 7, 8 log sheets. I already had 15 hours by the time I reached high school. I did a walk for cancer research/premature birthing in infants, the summer before 9th grade and our community service teacher explained to me that if I can prove I did it, I would get the hours. You can start ASAP, if you'd like. I knew some people who submitted a form a week before graduating, to reach their hours. Dont be one of those people. I would stop by your guidance counselor's room and ask how it works for your particular school district.
