Is it difficult for international students to be accepted as an academic/research intern or a research assistant at The University of Texas MD Anderson Cancer Center?

MD Anderson has been consistently ranked as the #1 cancer hospital in United States by US News & World Report over the last couple of years. It is a major research hospital and there are tons of research projects. Having worked with radiologists on their research projects, I know for a fact that a lot of physicians at MDACC are into research, so there would be plenty opportunities for anyone who wants to do research here. I am a graduate research assistant here myself, also an international student from mainland China. It's hard to say that whether it is "difficult" to get accepted, which I guess depends on your credentials and other uncertainties. But yes, they surely take international student as research interns/assistants.

I would say that a significant proportion of our researchers and research staff at MD Anderson are international. So the issue of where you're from is not an issue, but rather your credentials, background, experience, and perhaps English skills. And of course, the availability of positions.
