    You stand a chance of getting in. First, importantly, your overall GPA is above their minimum, so they won't outright reject you on first glance. Second, and most importantly, your recent grades are very, very strong. It'll be those recent grades that get you in. Good for you doing so much better. I think you stand a strong chance of being admitted. You may want to use an essay to explain what happened: why you got poor grades with them, what happened that woke you up, why you're doing better now. Take the opportunity to show them that the grades you get now are indicitive of the type of student you are now.

Yes, probably. GPA isn't going to be the only factor in whether you get accepted again, they'll look at your recent grades, other activities you do, etc.
