Tell them every place you've lived from April of 2005 to the present day. They want you to go BACK TO 2005, not back any further than that.

I am interpreting your question something like "Should I state that I lived there from August of '03 until August of '06?" Then yes, state the actual dates. My company's application form has the same thing and people are confused about what to state for dates. The real purpose is so that we can perform background checks against an address, back to a certain time.
