Is this good to say in the email for my job application?

What can I say on the email because I want to request for a job application form?

LOL lets just use a teency weency bit of common sense here. You are going to get an application form by email, so the employer is NOT interested in your address at the start. You can fill in your address on the form. Just write a simple email saying: Hi, I would like to apply for the administration role as advertised, can you please send me the necessary application form. Regards Hawk Just curious Hawk, just how old are you? You seem to have led a very sheltered life if you can't email someone for an application form?
