  It costs a fortune to hire an RN to work in an outpatient office. For example, if she is offered $30 an hour, the owner / doctor can hire three medical assistants (which may get $10 an hour) instead. And new grad RNs are not experienced enough, as Diane stated, to do the triage on her own. This is when the patient calls and the nurse makes decision about what the patient should do. (Doctor don't have time to answer any calls personally). Should patient take care of the problem at home on her own? Should the patient's message be left for the doctor to return the call later? Should the patient not worry about it? Should the patient come in now? Should the patient come in a day or two? Should the patient go to an urgent care? Should the patient call 911 or go to ER right now? Difficult decisions and if anything goes wrong, it can be life or death. That sort of responsibility rests on an RN and that's why they get pay more than a medical assistant (or even a LVN / LPN) I do know a clinic that uses an NP to do the triage call. She gets paid even more but is able to prescribe on the phone (calling in a prescription into a pharmacy) if she thinks it's appropriate for patient.
