You can get your SSS contribution print-out through (1) SSS Office - where you will fill out a form (about 1/4 sheet) and presenting this to the counter together with your Digitized ID card (or E-1) and (2) go to then SSS website (www.sss.gov.ph) . Click Online Inquiry - you will be asked to fill out your SSS Number, Surname and Date of Birth (mm-dd-yyyy). Once you've entered this information, you can check your actual contribution, employer history and other information you need to know. However, there is a tendency for both to be off-line; I advise you to be patient. Good luck.

Why is there this requirement? I can see for a security clearance; but otherwise: aren't they asking if: 1) You are making extra money in an outside job or if you lied about your work history? Your question is more interesting than the answers (all good) you got, I think.
