How does YouTube determine whether or not show me an advertisement for a given video?

I've noticed it's not consistent. Sometimes I get an advertisement beforehand, and I watch the same video later and get no advertisement. Then, for the same video later, I may be given an advertisement but also the option to skip it after the first 10 seconds. What's driving these algorithms?

The Uploader can choose whether to have two ads, one at the beginning of the video and one at the end, or the, equally spaced out in the video. The ads given to you are chosen by the use of Cookies. Some of the ads are very long ( over 1min) and can be skipped, some are short (30secs or less) and cannot.
