  I would contact your HR department and if in the same building you work visit in person to obtain the "Return to Work Certification" form, refer to sample below. This is your company's form created by HR with sections to be completed by you and your doctor. If HR does not have a form, which I doubt, then ask the manager to write down specifically what your doctor needs to provide. In the future if out for more than 3 days obtain a doctor's note.
