I am having the same problem you are. I opened the pdf file and was unable to input any information on it. Sorry.

The pdf sheet that is the application will let you fill in the blank spaces. (just click on the empty spaces - works fine for me). If that doesn't work for you, try to download a newer version of adode acrobat - available for free at http://www.adobe.com Fill in the application and save the finished sheet to your computer. Then attach the pdf (complete application) to your email before sending.
