  S: It's important to make it easy for the receiver to reply by making your request short, to the point, and stating its object in your first sentence which, I assume is to promote greater customer satisfaction resulting more sales. 1. An addressed, postage paid envelope is a must.; 2. Don't flood the receiver with a lot of questions, just 1 page of the most important. ie Delivery time,price, condition of merchandice when received, Meets customer's expectations etc. Many use multiple choices for answers which can be checked. 3. Allow space for customers' comments. 4. Introduce yourself and provide a feeback number or other means to contact you in event a customer needs to contact you with additiona questions and comments. It's important you state the receiver is a valued customer and would like their input so your concern can provide even better service to them. Thank the customer in advance for their response. In short: Put yourself in the customers' position and think what type survey is easy to respond to. This includes the importance of asking their comments and opinions on ways to improve yyour service.
