well, I would personally recomend someone who has known you a few year (the longer the better) and may have some knowledge of your work abilities. that goes a long way to making a good impression on an emploer

The best way to ask someone to be a job reference is to just merely tell them that you applied for a particular job or for various jobs and that do they think that they could give you a, good and honest reference. If they affirm, then tell them that they may expect a telephone call or calls concerning the job reference. Then thank them kindly. P.S. Make sure that you get their current telephone numbers and current address.

It really doesn't matter, to tell you the truth. This is based on experience too, by the way. Employers never ever call the references, it is just a way for them to gague what kind of people you know (or if you even know people). Notice how you have to put their phone number and who they are too. This shows the status level you hang out with. You should always put someone famous who is hard to get hold of.
