I wouldn't sweat it. Oyster cards can easily get mixed up (they all look the same, after all, and don't have photos on them). I'm sure it happens all the time.

Was there stored value on it (other than the trip she took)? If so, you and she should both go down to the office and ask them to sort it out. It was an honest mistake right? That's the trouble with these stored-value cards, there's no way to tell which is which, or what's valid etc. You can think you have your card with the monthly pass, and have no way to know until the fare inspector comes and says you don't.

It depends exactly what was on the card previously. AFAIK a standard PAYG adult oyster is transferable
