well in saltwater sea worms come from the sand and the fish go and eat them when they stick their heads out, and in freshwater the worms will get washed away into water when it rains, or come into water from erosion, or they dig and get into the water

There are lots of wormlike critters which live underwater, which fish have been eating since there were fish. (Start turning over rocks in a small stream, anything you find under there is great bait.) So when land worms get washed into the water (or cast in as bait) in front of a fish which has never seen such a thing before, the natural impulse is to eat it.
