You first go to college for an undergraduate bachelors degree. If I'm not mistaken your major is premed. Either that or you pick a major that is medically related and you're just considered "premed". After you graduate with your bachelors degree, you go onto med school and graduate with your doctorate. It's after med school that you begin your internship and get your license and all of the fun stuff that allows you to legally practice medicine.

In Europe you go to Medical School right off high school if you get admitted through entrance exams. In the U.S. first you get a Bachelor's Degree, preferrably in Biology. Then you take an exam to get admitted to Medical School. It's very competitive because doctors make a lot of money in the U.S.
