In most banks you can just give the money/cheque to the cashier and they will put in your account for you. The receipt is just for your peace of mind that the money has gone in. Alternatively you can fill the slip out yourself (some banks ask you to do this) you just need to put on your account number and sort code (on your card) and the amount your paying in. You will be sent a new card before the old one expires
