Does the post appear in the news feed as soon as I change the permissions? Or does it never appear in the news feed if it wasn't originally visible, but simply becomes visible on my timeline?

I think visibility follows the second rule you mention in your question details. IIRC, each Facebook post has customizable visibility. So you can go to that particular post you want to make visible, and change the settings. This, however, assumes that you can find the activity you intend to post. It can be tough for feed heavy apps like Quora.
