What do big, high charging SEO agencies do to their big clients' websites to improve rankings when they have big budgets?

I am talking about the clients who spend $10k + every month.blog As their sites are usually more difficult to rank when do the SEO agencies do? what is recommended by lot of people: On page: title, meta, some copyrighting, good layout, keyword use. Off page: blog, guest blogging, infographics, Press releases. I'm sure that there's more, but I can't think of anything at the moment. Even though those SEO methods are good I'm sure that it would be difficult to improve rankings on certain websites and for the website owner to feel that they have had a positive ROI. Or do they use other methods?
