How do you convert a design in FPGA to an ASIC? What are some companies that will do such a conversion for you and what do they need from you?

I have a working code on Altera DE2 Development and Education FPGA board. How do I get that converted to an ASIC prototype?

Answer:
Altera used to offer that with Altera Hardcopy, but they don't seem to have that anymore. Xilinx EasyPath is similar but not the same (they still use FPGAs).
