How to ask a girl out that you never exchanged a word with?

Okay, there is a girl in my school like always that can't get out of my mind, next week in school is also my las t in this school and maybe the last few hours i'll be able to see her and my last chance. We exchanged a few looks but that's it, it would be a lot easier if we were going in the same class but we're separated and i see her only durring breaks, when she goes to gym class and when she goes home. I tried to contact her on facebook but she didn't reply and ignored my friend request. I started to like this girl when she pointed her finger at me smiling with her friend because when i was walking by the hall i was eating the puff pastry and i had a little bit over my mouth, you know... What should i do?!?!?! This is the last time i'll ever see her(probably) and i'm deciding should i leave as a pussy or as a man, if i don't ask her i will never, but how should i approach her and talk to her, should i ask her directly would she like to go out with me????? Please do understand that i exchanged a look with her not a word:D Okay, i want to approach her without being a total creep, here's the scenario of me approaching to her: Le me going to school and i see her coming from gym and i fly to her as fast as i can so i can start a conversation with her while she is not in company so i wouldn't feel uncomfortable. I approach her to shake hands and i say my name expecting her to tell me her name too(even though i know it) and i directly ask her, "listen it's last week of school now so i was wondering if you, ugh... i kind of like you for a while now but i didn't know how to approach you, but i wanted to ask you would you like to go out with me on saturday or maybe go for some coffee?" Please give me serious answers. Ask her out or start a meaningless conversation? By the way i'm 18 and she's 16.

I wouldn't go all rehersed on this. Just casual and start up a conversation. Don't shake hands as it's kind of formal. Start off something like 'Hey I'm <insert name here>' Give her a minute to reply with something. But don't wait for ages if she doesn't answer. Start off with an open question like 'So how's it going?' Make it seem casual like you just struck up a random conversation when you approach her, not like your aiming for her. After you've had a fairly decent conversation, ask her if she's interested in going to get some coffee on <insert day here> don't ask anything big. Just casual coffee :) Good luck ;)
