Who fills out reference questions page on application: applicant, previous employer or leave it blank?

There is a reference request page in my job application. dates of employment, eligibility for rehire, skills rating, and questions about my job performance. I have no idea who fills it out. Do i ask my previous employer to answer those reference questions, do i answer them or do I leave it blank for when HR calls my references and fills it out with my previous employers answers?? This is my first time job hunting for my career in health care :)

LOL, you don't fill out your own reference form or call to get your own reference information. Put the employer name, supervisor, and contact information and leave the rest blank.
