Honestly I can't see why you're not suitable for that job given your experience. All I can try give you some help on the enthusiasm is to just be positive about everything. I'm not a very confident person, the last few months I've had my first ever job interview (I'm 18 too) and a couple of others and all I can say I tried to do was just be as positive as you can about everything. If they say "Oh you've only done volunteer work" try you're best to not look annoyed or apologetic but take that and twist it into a compliment. Talk about how much you love doing that work that you enjoyed gaining experience in it even if it was unpaid but now you feel prepared and ready for applying these skills into a professional establishment. Keep smiling and don't doubt yourself. Take each interview as an experience and something to build on, that's what I did and I've got a part-time job now. So good luck and even if it doesn't work out it's probably for the best and you'll have got interview practice which is always good =)

I know you know that McDonalds isn't a good place to work in, but if you really want to get somewhere with your culinary skills and you think that you would want to spend a LIFETIME working in this area, then I suggest you work in a better place. Different companies according to their different class/stature view others in a different view. In some cases you don't HAVE to be over enthusiastic. Try going for a restaurant, even a small local one instead of an industrial fast food joint. Dress modestly, and act modest and conformed, that's how you SHOULD act in the real world, not in McDonalds. The only reason I say that you should reach higher is because you seem like you have a lot of experience ! And an additional tip I give you is to say your experiences like you OWN THEMM. Don't say "I volunteered at this fair two months ago" say "I participated in the ______ fair for the (children in need?) and I was the assistant for the chef there". something like that. DON'T LIE, but you know, add more details, make it more elaborate for them to see.
