Where to find School application form?

Provisional License Authorized School Event: Where do I find the form?

• Ok I know you can't drive any minors if you yourself are a minor within the first year of getting your license without an adult in the car. My question is this; I know if its for an authorized school event you may drive someone else even with a provisional if you get a note signed by the dean/ principal / parents "Schooling or school–authorized activity. The note must be signed by your school principal, dean, or his/her designee." (http://www.dmv.ca.gov/dl/dl_info.htm#FIRSTYEAR) This is all ok with me, I need to know where do I get the legal form? I've searched the web / dmv / school website, and can find no such official document. Is it merely something I write/type myself? Please if you know where the document is, post a link; I don't need any more regulations even if my diction was not 100% accurate. I know for a fact this is possible, I've had friends do it, and I'm looking on the dmv.ca.gov website right now, I just don't know where to find the "note". Anybody know? Thanks again.

  Are you looking for something like authorized student driver form? It may not be exactly matching, but the one from the site below might be helpful.. you might download it and print and then wipe out the name on top to use your own school's name, etc... Good luck !
