How would you set long term and short term goals?

My entire life all I have had my mind set on is graduating from high school furthering my education by going to college for a nursing degree and then be working in that field to provide for my family. My question is how do you set a five year goal and further when you havent even thought of further goals. I have an assignment for school to do this and i have to add detail on these goals like either educational or career goals. The only goal i have in mind for a long term goal now is to receive a bachelors in terminologist. Im going to school now to recieve my associates degree in health administration. Anyone have ideas or advice about setting long term goals?
