How to record volunteer work for Medical School?

• Hello, I was just wondering how would medical schools know what, when, and where you did volunteer work or were employed in the healthcare field? I myself will be starting either volunteer or a job at a hospital or perhaps both and was wondering if there is some type of form that one records their volunteer work or jobs they have taken at a hospital for gaining experience in the field?
