Go to theoffice of the Assistant Commissioner, Saidapet Division of Civil Supplies Office situated near Saidapet Railway Station(I really forgot the name of the street)(whixh is in the first floor of a a corner building), where you can get an application form, fill it up & hand over there itself. After your address enquiry by a staff to your house, they will issue ration card. You should not have cancelled the ration card at Madurai,instead you should have surrendered it there and get a surrender certificate, then it should be easy for you now to get a new one here at Chennai.
