What on earth do I get my boyfriend for his 21st birthday next week?

I'm paying for his next tattoo as a present but he hasn't booked it yet, I want to give him something on his birthday he can open :/ I've bought him everything I could think of for christmas so I have no idea what to buy him, we've been together for 3 years & he pretty much has everything he would want. help?
