Why has the icon for Internet Explorer suddenly changed?

I've been using Internet Explorer 6 for about a year. I use to be able to start multiple instances by clicking on the icon, but can't do that anymore. I don't know what changed. SO, I downgraded to IE 5.5. That didn't help. Any suggestions?

The 'e' icon for html files have lost and i can see only a different icon. i tried to see if the default is changed or not, but it is still shows the default icon for all. How can i change it?
